In straightforward language, it explains the steps you can take to protect your future interests and make sure that things will be done in the way you would want - should you ever lose the ability to make these decisions for yourself. Unfortunately, printed copies are no longer available, but it can be downloaded from the link below. It is an A5 booklet, 40 pages.
