St George and Sutherland Live at Home Programs offer five main types of community care services:
- Case management, supporting people with complex care needs
- Respite care
- Social and peer support for people with a disability
- Healthy at Home and Compacks, helping frail older people avoid unnecessary episodes in hospital and people who have just come out of hospital.
Case management
This service organises and coordinates community care services for older people and people with a disability who have multiple health problems and need a lot of help to stay living at home, and the people who care for them.
Our skilled staff visit the person at home to assess and discuss their needs and then organise the range of care services that will help them live at home as confidently and independently as possible. Services can include help with showering, dressing, community nursing, home maintenance, housekeeping etc. We advocate on the person’s behalf with other service providers, provide a single contact point for them to ring if problems or changes occur and keep in touch to see how they are going. We currently support around 160 people through this service.

Respite care programs
We offer several respite care programs for carers (usually these are family members or friends) who are looking after an older person or an adult or child with a disability. Essentially we arrange to care for the person they look after, so that they can take a regular planned break. Typically this would be for a few hours each week.
Depending on what would work best, we organise for the person to be looked after in their own home, at a home-like cottage, or to be taken out for a few hours. We currently assist around 100 carers in this way.
| Eligibility | Family members or friends who are looking after an older person or adult with a disability or child with a disability at home. Also older people (aged over 65 or, if Aboriginal, over 45) who are caring for an adult son or daughter who has a disability. |
| Where | Hurstville, Kogarah, Rockdale and Sutherland local government areas. For carers of children aged 5-18 with a disability, those in the City of Sydney, Botany, Randwick, Waverley and Woollahra. |

Social and peer support
Through this service we assist people with a disability (including children) who need support to create or maintain social and peer connections within their communities. We organise adult groups, kids clubs, outings to the movies, bowling and other activities. We currently assist around 100 people in this way.

Healthy at Home and Compacks
Through the Healthy at Home service we help older people who have multiple health problems who, with quick access to the right care services in their own homes, can avoid having to go to hospital in a crisis. Through the Compacks service we assist people who have just come out of Sutherland or St George Hospitals, for up to 6 weeks.
We assess the person's needs in conjunction with their GP or the hospital and then organise the health and community services they require to help improve their condition or while they recover from being in hospital.
| Eligibility | Frail older people aged 65 and over (45 and over if Aboriginal) who live in Hurstville, Kogarah and Rockdale local government areas. People who are about to be discharged from St George or Sutherland Hospitals who need care services while they recover. |
| Fees | None. However, fees may be charged by some of the services organised for the person. If so, the amount charged will be adjusted according on their ability to pay. |
| Languages | We will organise interpreters if needed. We will also try to organise services that use bilingual staff, as appropriate. |
